Overview
We are seeking a dedicated and compassionate Care Manager for our client. The ideal candidate will have a strong background in care management, particularly within nursing homes, care homes, or home care settings. This role is essential in overseeing the delivery of high-quality care to residents, ensuring that their needs are met with dignity and respect. The Care Manager will play a pivotal role in developing care plans, administering medication, and leading a team of healthcare professionals to provide exceptional support for individuals with dementia and other care needs.
Responsibilities
- Manage the day-to-day operations of the care facility, ensuring compliance with regulatory standards and best practices.
- Develop and implement individualised care plans tailored to each resident’s specific needs and preferences.
- Oversee medication administration and ensure that all staff are trained in proper medication management protocols.
- Provide leadership and support to nursing staff, fostering a collaborative environment that promotes high standards of care.
- Conduct regular assessments of residents’ health and wellbeing, making adjustments to care plans as necessary.
- Liaise with families, healthcare professionals, and external agencies to coordinate comprehensive care services.
- Ensure that all documentation related to resident care is accurate, up-to-date, and compliant with relevant policies.
- Facilitate training sessions for staff on dementia care techniques and best practices in the field.
Qualifications
- Proven experience in a managerial role within a nursing home or similar setting is essential.
- Require CQC Level 5
- Strong knowledge of dementia care principles and practices is highly desirable.
- Relevant qualifications in nursing or health management are preferred.
- Excellent leadership skills with the ability to motivate and guide a team effectively.
- Strong organisational skills with attention to detail in managing care plans and documentation.
- Exceptional communication skills, both verbal and written, to engage effectively with residents, families, and staff members.
- A compassionate approach to caregiving with a commitment to enhancing the quality of life for residents.
