Job Title – Care Worker / Support Worker / Domiciliary Care Worker
Department – Adult Social Care
Purpose of the Job – Working with individuals, carers and families, often in difficult social circumstances, the Social Care worker will undertake front line support
As a member of a multi-disciplinary, multi-professional team the Social Care worker will work with adults, young people, carers and their families to enable them to live to their maximum level of independence providing a range of interventions and direct support.
The Social Care worker will be responsible for providing direct care, including personal and practical assistance to service users.
The role requires an awareness of relevant legislation, processes and procedures and an understanding of the issues relating to the service user group with the ability to provide personal or practical interventions in accordance with laid down procedures and regulatory frameworks.
Areas of Responsibility – Act as front line worker providing person centered support to individuals on behalf of the social work service particularly noting details with regard to safeguarding / risk issues and use decision making processes to either forward to the Registered Manager for social work intervention and specialist assessment.
Provide information support, guidance, advice and services to adults, young people, carers and their families and to ensure that issues of concern are addressed appropriately and in a polite, professional and timely manner, passing any concerns onto management.
Follow assessments as allocated including multiagency, multi-professional and other as required in line with legislative and policy guidance and requirements for service area.
Assess and manage levels of risk appropriate to role.
Ability to recognize and escalate more significant risks to Line Manager.
Deliver front line support as outlined in the assessment / support plan and any individual health plans appropriate to the individual.
Develop and deliver session plans based around assessed needs which focus on the individual.
Have a thorough understanding of specialised methods, techniques and processes related to health & social care and make best use of community assets to provide a response.
Work in different settings across the borough as required appropriate to role.
Knowledge. Skills & Abilities – Practical experience acquired through a combination of training and working in relevant area with clear understanding of applicable standards and regulations.
